چکیده

Dear Editor, the recent report on “Isoproterenol infusion in septic shock” is very interesting (1). Wiramus et al. concluded that “use of isoproterenol was associated with an increase of tissue oxygen saturation (1).” In fact, there are some issues of isoproterenol use to be discussed. First, the increased heart rate due to infusion is common (2). On the other hand, the occurrence of paradoxical bradycardia can also be observed (3). In addition, the possibility of occurrence of isoproterenol-induced myocardial infarction should be kept in mind (4). The fatal case due to the infusion is reported in the literature (5). To prevent cardiac toxicity due to isoproterenol infusion, careful evaluation before usage is required. REFERENCES

Ventricular Blood Flow, and Epicardial ST-segment Elevation During Coronary Occlusion in Dogs

The effect of p-chlorophenoxyisobutyrate (CPIB) on ST-segment elevation in epicardial electrocardiographic recordings was studied during coronary artery occlusion in dogs. Occlusion alone raised the sum of ST-segment elevations (2ST) to 26 ± 6 mV (mean ±SEM). Intravenous (i.v.) administration of CPIB 30 min before re-occlusion reduced 2ST to 14 ± 3 mV (P < 0.03). A continuous i.v. infusion of i...

Effects of p-chlorophenoxyisobutyrate on myocardial free fatty acid extraction, ventricular blood flow, and epicardial ST-segment elevation during coronary occlusion in dogs.

The effect of p-chlorophenoxyisobutyrate (CPIB) on ST-segment elevation in epicardial electrocardiographic recordings was studied during coronary artery occlusion in dogs. Occlusion alone raised the sum of ST-segment elevations (sigmaST) to 26 +/- 6 mV (mean +/- SEM). Intravenous (i.v.) administration of CPIB 30 min before re-occlusion reduced sigmaST to 14 +/- 3 mV (P less than 0.03). A contin...

Isoproterenol infusion and microcirculation in septic shock

INTRODUCTION Our study primarily aimed at investigating the effect of isoproterenol infusion on tissue oxygen saturation in patients with septic shock. The secondary aim was to assess the relation between cardiac index, central venous oxygen saturation and tissue oxygen saturation. METHODS This retrospective study was conducted from December 2010 to March 2012. We included 14 consecutive pati...

Correlation of maximum aortic and carotid flow acceleration in chronically instrumented dogs.

Peak flow acceleration measured in the common carotid artery was compared with peak flow acceleration measured in the ascending aorta of five chronically instrumented dogs. Electromagnetic flow sensors, myocardial electrocardiographic leads, right atrial catheters, and coronary occluders were implanted through an incision of the chest at the fourth intercostal space and a ventral midline incisi...

Non surgical coronary artery recanalization in acute transmural myocardial infarction.

In 41 consecutive patients with an acute transmural myocardial infarction (AMI) admitted within 3 hours after the onset of symptoms, we tried to recanalize the occluded coronary artery by an intracoronary infusion of streptokinase (SK) (2000 units/min). SK infusion was preceded by (1) an intracoronary injection of 0.5 mg nitroglycerin to rule out coronary artery spasm, (2) an attempt to recanal...
